Pain Clinic Financial Information

Because we accept referral patients only at the pain clinic, the referring physician will give us your insurance information and, if necessary, obtain an authorization for the initial visit. When our office calls to schedule your appointment, they will discuss your insurance coverage with you. Many insurance companies require a co-payment be made at the time of service. We require you to make this payment when you come for your initial visit. We accept cash, personal check and the following credit cards: Visa, MasterCard, American Express and Discover. If you do not pay your co-pay at the time of your visit and we must bill you, we will add a service charge of $8.

Our charge for the initial evaluation varies depending upon the complexity of your situation. Charges for blocks are more because it involves the use of an operating room and an anesthesiologist, as well as being a more complicated procedure. We participate with most types of insurance so your portion will vary according to your insurance carrier.

Your bill is due and payable when the service is rendered. As a courtesy, Brevard Anesthesia Services, P.A. will submit your bill to the insurance before looking to you for payment. We typically allow two billing cycles (sixty days) for the insurance company to pay. You will receive statements throughout this process. If your insurance has not paid after thirty days, we ask you to contact them and/or our billing office (321-723-4723) to determine how you may assist in expediting payment. After sixty days, we will look to you for payment of your outstanding bill. As such, anything you can do to expedite this process will serve everyone's best interest.

Some carriers do not cover some procedures, or do not consider anesthesia services to be medically necessary for them. In these instances, or if you do not have insurance, we ask that you contact our billing office before your appointment to make payment arrangements. The sooner you contact us, the more options we will have if you will need to make payment arrangements.

Because we perform some blocks at the Health First Center for Outpatient Pain Management and others at the Melbourne Same Day Surgery Center, patients receiving procedures at those facilities will also receive a bill from them.
